To ensure you get the best care possible and to ensure that insurance billing goes smoothly, we ask that all patients complete our online intake and consent forms before their first appointment and then again each year. It's crucial to complete these - without them, we might have to cancel your appointment, and we won’t be able to prescribe any medications.
